HealthMedia, Inc. (HealthMedia®) is committed to the privacy and confidentiality of personal information. HealthMedia® recognizes that the security of your health care information is of great concern to you. Any health information you have provided to HealthMedia® that is linked with an identifier, such as your name, is defined as personal health information.

To protect your personal privacy and confidentiality, HealthMedia® operates secure websites and other applications (see "Website and Data Encryption") (which, collectively are referred to as the "Website"), has adopted use and disclosure policies (see "Use and Disclosure Policies"), and has implemented various security measures (see "Security Measures"). Our commitment to your privacy is reinforced by our adherence to the following privacy policy.

It is the policy of HealthMedia® to not voluntarily disclose your personal health information to your employer, insurance company, or any other third party without your specific authorization or as required by law.

HealthMedia® CounselorAssist™
For follow-up intervention, your sponsoring organization MAY offer HealthMedia® CounselorAssist™. HealthMedia® CounselorAssist™ allows your counselor to obtain access to your program materials so that they can better assist you. If this program is being offered to you, your sponsoring organization will need your authorization to access your personal health information.

If HealthMedia® CounselorAssist™ is being offered, you will be asked to review and "Accept" the authorization for sharing your personal health information with your counselor. If you do not wish to share your personal health information with your counselor, please select "Do Not Accept" as part of the same authorization question, but you will not have the benefit of counselor access to your personal health information.

HealthMedia® STEP BY STEP™ Application
HealthMedia® may provide the HealthMedia® STEP BY STEP™ Application (the "Licensed Application") for use and download on your mobile device and/or computer. Data collected through your use of the Licensed Application is governed by this Privacy Policy. Your running/walking data can be generated on your mobile device by the Licensed Application automatically if you select that functionality. You may choose to let the Licensed Application automatically track your running/walking data for you or you may enter your running/walking data manually on your mobile device. If you choose to have the Licensed Application track your running/walking progress for you automatically, the Licensed Application will use the Global Positioning System (GPS) function on your mobile device to track your distance. Therefore, in order to use the automatic tracking function of the Licensed Application your GPS function must be enabled on your mobile device.

In addition, the Licensed Application is designed to work in tandem with information you submit and/or upload to the Website. This includes information that is automatically generated by certain supported devices, such as pedometers and blood pressure monitors as specified in the Licensed Application, and uploaded to the Website by you. Therefore, certain information you submit to the Website will be downloaded to your mobile device. Such information does not include your personally identifiable information, but may include such information as your height, weight, cholesterol levels, blood pressure, goals (e.g., steps walking, time running, calorie intake, etc.), and summary information shown in data charts or graphs.

Website and Data Encryption
To support our commitment to protecting your privacy and ensuring confidentiality of all information you provide, HealthMedia® uses VeriSign Secure Site digital certificates to provide authentication of HealthMedia®, enabling end users to verify connections to the Website and to communicate via SSL encryption, thus further protecting confidential information from interception and hacking. VeriSign (www.verisign.com) is a leading provider of Internet trust services. Further, all personally identifying data is stored in an encrypted database.

Data collected via the Licensed Application is transferred in real-time to the HealthMedia® database in an encrypted format via your mobile service (including with the use of WiFi or other method of connectivity). However, at times when your mobile device loses its connectivity to your mobile services, or such mobile functionality is disabled or turned off, data collected via the Licensed Application will be stored on your mobile device in a secure but unencrypted format. HealthMedia® has designed the Licensed Application to require you to enter a password to gain access to your information stored within the Licensed Application on your mobile device, HealthMedia® encourages you to take advantage of the additional security features that may be offered on your mobile device, such as the ability to set up additional passwords to gain access to the features, applications and data therein. In addition, when you sync your mobile device to iTunes, you may have the option to backup and store your data on your computer. HealthMedia® encourages you to backup your data, and to secure and utilize encryption on your computer and your mobile device when doing so.

Use and Disclosure Policies
The following paragraphs disclose information gathering and dissemination practices for this Website.

By participating in a HealthMedia® program you will receive a tailored behavior change intervention guide and follow-up communications. In order to provide you with an accurate and useful tailored personal guide, HealthMedia® asks for the minimally necessary information which includes your name, address, and relevant health history along with information about lifestyle behaviors such as smoking and exercise, physical measurements such as height, weight, and blood pressure, and mental health topics. Your answers form the basis of the personal guide that is unique to you. In conjunction with the personal guide you may also receive follow-up newsletters and emails. Email communications will be sent to the email address you provide. These communications may contain information about your condition or disease state. It is the policy of HealthMedia® to not voluntarily disclose your personal health information to your employer, insurance company, or any other third party without your specific authorization or as required by law.

During sign-up, you will be given the opportunity to indicate your preference to receive program information or to participate in future research studies. If you indicate a preference to receive information, HealthMedia® will use your contact information to send you information about HealthMedia®, material about related products, and invitations to participate in research studies to assess the effectiveness of the HealthMedia® programs.

Regardless of your initial response in the questionnaire, you may change it. The HealthMedia® Website provides you the opportunity to update your response to the question "May we contact you in the future with program information or for research purposes?" To update your response to this question, select the My Account link in the top navigation bar. Then select Update Your Contact Information. This will allow you to change your answer selection.

HealthMedia® will create aggregate data results for dissemination to other organizations and for research purposes. THIS AGGREGATE DATA WILL NOT CONTAIN ANY INFORMATION WHICH DISCLOSES YOUR IDENTITY. In this case, your data is combined with data of other participants to determine an overall profile of a population. These data become statistics and may be used by HealthMedia® or other organizations to understand the needs and characteristics for selected populations.

HealthMedia® uses your IP address only in the event a Web server problem requires diagnosis and to administer the Website. HealthMedia® will not distribute your IP address to any third parties.

Linked Sites
This Website may have links to other websites that contain information HealthMedia® believes may be helpful to you. Please be aware that HealthMedia® does not control other websites and that, in any case, this Privacy Policy does not apply to those websites. You are encouraged to read the privacy policy of every website you visit.

Security Measures
This Website has security measures in place to protect against the loss, misuse, and alteration of your information under the control of HealthMedia®. These measures include:

Authorized Access: You must be registered to use this Website. The registration process requires a sponsoring organization identifier. Once you are registered, you will use a unique user ID and password to enter the Website. Your password is not accessible by HealthMedia®. If you lose your password, it cannot be retrieved. At your request, HealthMedia® will issue you a new password.

Authorized Access to the Licensed Application: In order to activate the Licensed Application on your mobile device you must first enter a keycode provided to you on the Website. In addition, you must create a password that must be entered before you may use the application on your mobile device. HealthMedia® reserves the right to require you to change or update your password at regular intervals.

Authentication: HealthMedia® uses VeriSign's Secure Site Services to provide authentication, trust, and SSL encryption. This allows Website visitors to verify HealthMedia® with a trusted third party organization, while enabling communication with HealthMedia® Websites using secure, encrypted channels.

Cookies: A cookie is a piece of data containing information about the user. Usage of a cookie is not in any way linked to personally identifiable information while on the HealthMedia® Website. HealthMedia® uses a session cookie to ensure that your session remains secure by maintaining an identifier unique to your session and automatically logging you off after a period of inactivity. The session cookie self-terminates after a set expiration time or when you close your browser. Persistent cookies are not used.

Encryption: HealthMedia® employs Secure Socket Layer (SSL) encryption in the use of its products. The HealthMedia® Website will support down to 128-bit encryption. Personally identified data is stored in an encrypted database.

Children's Guidelines
HealthMedia® is committed to protecting the privacy of children. Other than HealthMedia® TEEN SUCCEED™, PRODUCTS ARE NOT INTENDED FOR USE BY ANYONE UNDER THE AGE OF 18. HealthMedia® TEEN SUCCEED™, is intended for use by teens ages 13-19 years old. In the event the Website recognizes that anyone under 18 (or in the case of HealthMedia® TEEN SUCCEED™, anyone under 13), has attempted to send personal information, the session will be aborted and an appropriate notice posted to the user.

Corrections and Updates
To make corrections and updates to your personal information, go to My Account after you have logged in. You will be able to change your name, birth date, mailing address, phone number, your option to be contacted by HealthMedia® with future program information or for research purposes, your email address, or your password.

Changes to This Policy
HealthMedia® reserves the right to change this Policy at any time and in its sole discretion. If the Policy is changed, the new Policy will be posted here, along with its revision date, and all information collected following the posting of such changes will be subject to the new Policy. However, before HealthMedia® discloses your previously collected personal health information in a materially different manner from that which you authorized, HealthMedia® will first secure your affirmative consent for such use or disclosure. Absent such consent, HealthMedia® will disclose your personally identifiable information in accordance with the Policy in effect at the time the information was collected.
